Reportable Diseases

The Erie County Department of Health is responsible for investigating diseases and conditions identified on the Pennsylvania Department of Health’s list of reportable diseases.

Reports can be submitted using the following forms:

For health-care practitioners and health-care facilities, all diseases are reportable within five work days, unless otherwise noted. Some diseases require reporting within 24 hours.

For clinical laboratories, all diseases are reportable by the next work day, unless otherwise noted.

All disease outbreaks and/or unusual occurrences of disease are reportable within the Commonwealth. Note that local jurisdictions may require reports of additional conditions not listed here within their jurisdictions.

Certain broad categories such as Food Poisoning Outbreak should be construed to mean all such illnesses. Ehrlichiosis should be reported under the authority of “Rickettsial diseases.”  West Nile infections and yellow fever should be reported under Arborviruses.  Cyclospora is locally reportable.
