The Erie County Department of Health collaborates with different community partners to develop the county’s:

Community Health Needs Assessment
Every few years, this assessment is done to gather information about the community’s current health status, needs and issues by asking about people’s health experiences and environments.

Community Health Improvement Plan
Answers from the assessment are then collected and analyzed to develop this plan, which is used to prioritize future health programs and services for the public.
